This article explores buprenorphine treatment and a stepped care model. Although counseling is a required part of office-based buprenorphine treatment of opioid use disorders, the nature of what constitutes appropriate counseling is unclear and controversial. This article reviews the existing literature on the role, nature, and intensity of behavioral interventions in office-based buprenorphine treatment.

Kathleen M. Carroll, Ph.D.
Dr. Carroll is a professor in the Department of Psychiatry, Yale University School of Medicine. Dr. Carroll is the author of over 300 peer-reviewed publications as well as numerous chapters and books. Her research has focused on the development and evaluation of behavioral treatments and combinations of behavioral therapies and pharmacotherapies to improve treatment outcomes for individuals with substance use disorders.
Roger D. Weiss, M.D.
Roger D. Weiss, M.D., is Professor of Psychiatry at Harvard Medical School. His career has focused on treatment and clinical research with substance-dependent patients, particularly those with co-occurring psychiatric illness. He has authored over 300 articles and book chapters as well as 2 books, and has participated in a number of prominent national committees.
L.A. Rankin is a social worker with experience in many different settings with a variety of clients. She has worked with dementia and Alzheimers patients, dual diagnosis MH/MR, in a battered women’s shelter, and a rape crisis center. She also has 11 years of experience as a child protective social worker, where she earned certificates in domestic abuse/family violence and substance abuse.

After taking this course, you should be able to:
Acknowledge that buprenorphine is effective, but is has a significantly higher drop out rate than methadone.
Identify that some patients require psychological intervention, and that denying this service poses great risk to those patients.
Realize that there is conflicting data as to whether psychological intervention is effective, but that high quality medical management is effective
